As part of a three-year Diabetes Initiative, the Foundation, in conjunction with health literacy experts, has developed a diabetes self-management guide, Living with Diabetes: An Everyday Guide for You and Your Family. The guide, which is made possible through an educational grant from Novo Nordisk Inc., covers diet, exercise, monitoring blood sugar, insulin, and pills. Offered in English and Spanish, the guide is written in a conversational tone at a fifth-grade reading level and includes photographs of patients with diabetes and their families. It gives practical tips and concrete examples of successful approaches. The guide has been tested with over 200 patients, and initial feedback has been extremely positive. The Foundation hopes that the diabetes self-management guide will serve as an innovative model for patient education and empowerment. | |
